Responsibilities
- Perform comprehensive telephone and in-person triage assessments for patients presenting with acute and chronic conditions.
- Apply clinical decision-making skills to prioritize patient care based on urgency and severity of symptoms.
- Document all interactions, assessments, and care plans accurately and promptly in the Electronic Health Record (EHR).
- Collaborate closely with physicians, nurse practitioners, and care coordinators to ensure seamless continuity of care.
- Educate patients and families on disease management, medication adherence, and preventative health strategies.
- Manage high-volume call volumes while maintaining a high standard of empathy, professionalism, and de-escalation techniques.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in the State of Arizona required.
- Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications.
- Minimum of 2-3 years of progressive nursing experience, preferably in a triage, urgent care, or call center setting.
- Strong proficiency with electronic medical records (EMR/EHR) systems.
- Exceptional verbal and written communication skills with the ability to de-escalate sensitive situations.
- Demonstrated ability to think critically and make sound clinical judgments under pressure.
