Responsibilities
- Perform comprehensive remote patient assessments and monitor vital signs using secure digital health tools.
- Document patient care plans, daily updates, and medical history accurately in our electronic health record (EHR) system.
- Communicate effectively with patients, families, and medical staff via secure video conferencing and telehealth platforms.
- Provide emotional support and guidance to patients regarding their daily living activities and health management.
- Report any urgent changes in a patient's condition or potential safety risks to the on-site nursing supervisor immediately.
- Ensure strict adherence to HIPAA regulations and company privacy policies at all times.
Qualifications
- Current, valid CNA (Certified Nursing Assistant) license in the state of Ohio.
- High school diploma or GED equivalent required.
- Proficient in using computers and video conferencing software (Zoom, Teams, or similar platforms).
- Strong interpersonal and communication skills with the ability to build rapport remotely.
- Ability to work independently while maintaining high standards of patient care.
- Valid driver's license and access to reliable transportation (for occasional off-site training or documentation pick-up).
