Responsibilities
- Conduct Virtual Patient Assessments: Utilize secure video conferencing tools to perform initial screenings and ongoing health check-ins for patients.
- Monitor Health Metrics: Review patient-reported data, vital signs, and symptoms remotely to identify potential issues before they escalate.
- Manage Patient Records: Maintain accurate, up-to-date electronic health records (EHR) and documentation in compliance with HIPAA regulations.
- Coordinate Care: Communicate effectively with physicians, nurses, and family members to ensure a seamless continuum of care.
- Provide Health Education: Educate patients and caregivers on disease management, medication adherence, and wellness strategies via remote channels.
Qualifications
- Certification: Active and unencumbered Certified Nursing Assistant (CNA) license in the state of California.
- Experience: Minimum of 1-2 years of experience in patient care, preferably in a home health or telehealth setting.
- Certifications: Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification.
- Technical Skills: Proficient in using computers, tablets, and video conferencing software (Zoom, Teams, etc.).
- Communication: Excellent verbal and written communication skills with the ability to convey complex medical information clearly.
- Reliability: Proven ability to work independently in a remote environment with strict adherence to deadlines.
