Responsibilities
- Conduct virtual patient assessments and monitor vital signs remotely to ensure patient stability.
- Communicate effectively with physicians, nurses, and patients to coordinate care plans and follow-up appointments.
- Maintain accurate, up-to-date electronic health records (EHR) for all assigned patients.
- Provide emotional support and health education to patients and their families via phone and video conferencing.
- Respond promptly to patient inquiries and escalate urgent medical concerns to the on-call clinical team.
- Collaborate with the care coordination team to identify opportunities for improving patient outcomes.
Qualifications
- Active and unencumbered CNA (Certified Nursing Assistant) license in the State of Texas.
- Current Basic Life Support (BLS) certification from the American Heart Association (AHA).
- Reliable, high-speed internet connection and a dedicated workspace free from distractions.
- Strong computer literacy and proficiency with electronic health record systems.
- Excellent verbal and written communication skills with a focus on empathy and clarity.
- Ability to work independently and manage time effectively in a remote environment.
- Must be willing to submit to a background check and drug screening.
