Responsibilities
- Conduct virtual health assessments and medication management support via HIPAA-compliant telehealth platform
- Provide remote companionship and emotional support to elderly and chronically ill patients
- Coordinate care plans with RNs and physicians to ensure continuity of services
- Document patient interactions accurately in electronic health records (EHR)
- Assist patients with scheduling virtual appointments and transportation coordination
- Respond to patient inquiries with empathy and clinical accuracy
- Collaborate with interdisciplinary teams to address complex care needs
Qualifications
- Active Arizona CNA certification in good standing
- Minimum 1 year clinical experience in home care or geriatric settings
- Proficiency with telehealth software and electronic health records
- Exceptional communication skills with diverse patient populations
- Ability to pass comprehensive background check and drug screening
- Reliable high-speed internet and quiet home workspace
- Valid Arizona driver's license (for occasional field visits)
- Basic knowledge of chronic disease management protocols
