Responsibilities
- Deliver personalized in-home care including meal preparation, medication reminders, and mobility assistance
- Monitor client health status and document care activities using our secure remote platform
- Coordinate with family members and healthcare providers via virtual communication tools
- Transport clients to medical appointments and community events as needed
- Create and implement individualized care plans based on client preferences and needs
- Provide companionship and emotional support to promote client well-being
- Adhere to HIPAA compliance and agency protocols for all care documentation
Qualifications
- State-certified nursing assistant (CNA) or home health aide credentials
- Minimum 2 years professional caregiving experience
- Valid Wisconsin driver's license with clean driving record
- Proficiency with telehealth platforms and electronic documentation systems
- Exceptional interpersonal skills with ability to build trust with clients
- Ability to pass thorough background check and drug screening
- CPR/First Aid certification or willingness to obtain upon hire
- Flexibility to work occasional evenings and weekends
