Responsibilities
- Provide personalized in-home care including medication management, meal preparation, and mobility assistance
- Coordinate care plans via telehealth platforms and remote monitoring systems
- Facilitate virtual wellness check-ins and emergency response protocols
- Document client progress and care updates through our digital health portal
- Collaborate with healthcare teams during remote case conferences
- Transport clients to medical appointments when required

Qualifications
- Minimum 1 year professional caregiving experience or equivalent training
- Current CPR/First Aid certification
- Proficiency with telehealth software and digital health records
- Reliable transportation and valid NC driver's license
