Responsibilities
- Provide personalized in-home care including meal preparation, medication reminders, and mobility assistance
- Monitor client health status and document care notes via our secure remote portal
- Coordinate with healthcare teams through virtual consultations
- Assist with daily living activities while maintaining client dignity and independence
- Transport clients to appointments when required (mileage reimbursed)
- Participate in virtual care planning sessions with family members
Qualifications
- Minimum 1 year professional caregiving experience or CNA certification
- Valid Wisconsin driver's license and reliable transportation
- Proficient with telehealth technology and electronic documentation
- Ability to work independently while maintaining remote oversight
- Compassionate nature with excellent communication skills
- Successful background check and TB test clearance
- Flexibility to work some weekends and holidays
