Responsibilities
- Provide personalized in-home care including meal preparation, medication reminders, and mobility assistance
- Monitor client health status through virtual check-ins and document observations in our HIPAA-compliant portal
- Coordinate with family members and healthcare providers via telehealth platforms
- Transport clients to medical appointments using company-provided vehicles
- Assist with daily living activities while maintaining client dignity and independence
- Implement personalized care plans and adapt services to evolving needs
- Participate in remote care team meetings and continuous training programs
Qualifications
- Minimum 1 year professional caregiving experience or relevant certification
- Valid North Carolina driver's license with clean driving record
- Proficiency with telehealth tools (Zoom, Epic, etc.)
- CPR/First Aid certification (or willingness to obtain)
- Ability to pass comprehensive background check and drug screening
- Strong communication skills with both clients and remote support teams
- Reliable transportation for in-home visits
- Empathetic demeanor and crisis management training
