Responsibilities
- Provide in-home personal care assistance including bathing, dressing, and mobility support
- Monitor patient vital signs and report changes to clinical team
- Coordinate telehealth appointments and medication reminders remotely
- Prepare nutritious meals and assist with light housekeeping
- Transport clients to medical appointments using company vehicles
- Maintain accurate electronic health records via our remote portal
- Collaborate with RNs and therapists during virtual care conferences
Qualifications
- Minimum 1 year Home Health Aide certification (HHA/CNA)
- Current CPR/First Aid certification
- Reliable transportation and clean driving record
- Proficient with telehealth platforms and electronic documentation
- Ability to stand/walk for extended periods during home visits
- Clear TB test and background check
- Strong communication skills in English and Spanish preferred
