Responsibilities
- Conduct comprehensive telephone/virtual patient assessments using evidence-based triage protocols
- Collaborate with multidisciplinary teams to develop patient care plans
- Document clinical encounters accurately in EMR systems while maintaining HIPAA compliance
- Provide health education and crisis intervention to diverse patient populations
- Coordinate referrals and transitions of care across healthcare continuum
- Participate in continuous quality improvement initiatives
- Maintain emergency response readiness during after-hours shifts
Qualifications
- Active RN license in Virginia (or compact state)
- Minimum 2 years clinical nursing experience in emergency/urgent care setting
- BLS certification; ACLS preferred
- Proficiency with telehealth platforms (e.g., Teladoc, Amwell)
- Exceptional critical thinking and decision-making abilities
- Strong written/verbal communication skills
- Experience with Epic EMR systems preferred
- Ability to work independently with minimal supervision
