Responsibilities
- Deliver expert virtual patient assessments and triage services via telehealth platform
- Develop and implement individualized care plans for chronic condition management
- Coordinate interdisciplinary care teams and communicate with providers/clients
- Document clinical encounters accurately in EHR systems while maintaining HIPAA compliance
- Provide health education and medication management support to patients
- Monitor patient progress and adjust care strategies based on outcomes
- Participate in continuous quality improvement initiatives
Qualifications
- Active Georgia RN license in good standing
- Minimum 2 years clinical nursing experience
- Telehealth or virtual care experience preferred
- Strong assessment and critical thinking skills
- Excellent written and verbal communication abilities
- Proficiency with EHR systems and telehealth technology
- BLS certification required; ACLS preferred
- Ability to work independently in a remote environment
