Responsibilities
- Conduct comprehensive patient assessments via secure telehealth platforms and video consultations.
- Provide triage and clinical decision-making for a diverse patient population in a remote setting.
- Document patient interactions, diagnoses, and treatment plans accurately in EHR systems.
- Collaborate effectively with physicians, nurse practitioners, and care coordinators to ensure continuity of care.
- Provide patient education regarding disease prevention, medication management, and lifestyle changes.
- Respond to patient inquiries and urgent messages within established service level agreements.
Qualifications
- Active RN License in North Carolina (required).
- BSN preferred; Associate Degree in Nursing (ADN) accepted with relevant experience.
- Current BLS (Basic Life Support) and ACLS certifications.
- Proficiency with Electronic Health Records (EHR) and telehealth software (e.g., Zoom Health, Amwell).
- Strong communication and interpersonal skills with the ability to build rapport remotely.
- Ability to work independently and manage time effectively in a remote environment.
