Responsibilities
- Provide personalized care plans for elderly clients with complex medical needs
- Assist with daily living activities including mobility support, medication management, and meal preparation
- Monitor vital signs and document health status using electronic health records
- Coordinate with family members and healthcare professionals to ensure continuity of care
- Implement dementia-specific care techniques and memory support strategies
- Transport clients to medical appointments and social activities
- Maintain a safe, clean, and stimulating home environment
Qualifications
- Minimum 3 years professional caregiving experience with seniors
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) license required
- Valid CPR/First Aid certification
- Exceptional communication skills and emotional intelligence
- Ability to lift 50 pounds and perform physical care duties
- Reliable transportation and clean driving record
- Experience working with clients requiring memory care support
- Pass comprehensive background and drug screening
