Responsibilities
- Provide personalized in-home care including meal preparation, medication reminders, and mobility assistance
- Coordinate care plans via our proprietary telehealth platform for seamless remote monitoring
- Document client progress and health updates using our mobile-friendly EHR system
- Transport clients to medical appointments using company-provided vehicles
- Facilitate virtual wellness sessions for isolated seniors
- Collaborate with family members and healthcare providers through our secure communication portal
- Respond to emergency situations with certified first aid/CPR protocols
Qualifications
- Minimum 1 year professional caregiving experience or CNA certification
- Reliable transportation with valid AZ driver's license and clean driving record
- Proficiency with telehealth apps and digital care documentation tools
- Current CPR/First Aid certification (or willingness to obtain)
- Ability to pass background check and TB screening
- Empathetic communication style with seniors and their families
- Flexible availability including evenings and weekends
- High school diploma or equivalent required
