Responsibilities
- Perform comprehensive patient assessments via phone and digital platforms to determine medical acuity and urgency.
- Utilize clinical decision-making skills to recommend appropriate care pathways (e.g., ER, Urgent Care, Home Care).
- Document all patient interactions and triage decisions accurately and promptly in EMR systems.
- Coordinate patient transfers, referrals, and specialty consultations with medical staff.
- Provide evidence-based health education and resources to patients and families to promote self-management.
- Collaborate with physicians and care managers to ensure continuity of care for complex cases.
- Stay updated on current triage protocols, clinical guidelines, and regulatory requirements.
Qualifications
- Active, unencumbered RN license in Florida (or compact state) required.
- BSN degree preferred; minimum of 3-5 years of critical care, ER, or telemetry experience.
- Certification in Triage (e.g., CTRN, CPHQ) is highly desired.
- Strong communication skills with the ability to de-escalate patient concerns effectively.
- Proficient in Electronic Health Record (EHR) systems such as Epic or Cerner.
- Ability to work flexible hours, including evenings and weekends, as required by patient demand.
