Responsibilities
- Conduct thorough phone assessments of patients presenting with acute or chronic symptoms to determine the urgency of care.
- Apply clinical protocols and critical thinking skills to recommend appropriate home care instructions or treatment plans.
- Document patient interactions accurately and completely in electronic health records (EHR) systems.
- Effectively triage and redirect patients to the appropriate level of care, including Urgent Care, ER, or physician consultations.
- Educate patients and caregivers on medication management, symptom monitoring, and preventative health strategies.
- Collaborate with physicians and other healthcare professionals to ensure continuity of care and optimal patient outcomes.
- Manage high call volumes while maintaining a calm, professional, and empathetic demeanor.
Qualifications
- Active, unencumbered RN license in the state of Texas.
- Minimum of 2-3 years of experience in nursing, preferably in a triage, telehealth, or call center setting.
- Certification in Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS) is required.
- Strong computer literacy and proficiency with electronic medical record (EMR) systems.
- Excellent verbal and written communication skills with the ability to de-escalate tense situations.
- Ability to work independently while adhering to strict protocols and guidelines.
- High school diploma or equivalent required.
