Responsibilities
- Assess patient calls via phone and telehealth platforms to determine medical urgency and prioritize care needs
- Provide accurate clinical guidance to patients while maintaining compliance with healthcare protocols
- Collaborate with physicians and specialists to coordinate patient care pathways
- Maintain detailed electronic health records (EHR) with precise documentation
- Identify high-risk cases and initiate emergency response protocols
- Educate patients on symptoms, home care, and preventive measures
- Participate in quality improvement initiatives for triage processes
Qualifications
- Active RN license in the state of Colorado
- Minimum 3 years of clinical nursing experience with triage exposure
- BLS and ACLS certifications required
- Strong proficiency in electronic health record systems (e.g., Epic, Cerner)
- Exceptional critical thinking and decision-making abilities
- Excellent communication skills with diverse patient populations
- Ability to remain calm under pressure in emergency situations
- Experience with telehealth platforms preferred
