Responsibilities
- Assess and prioritize patient calls using evidence-based protocols to determine urgency and appropriate care pathways
- Delicate compassionate, accurate medical advice via telehealth and in-person consultations
- Collaborate with multidisciplinary teams to ensure seamless patient transitions through care continuum
- Maintain meticulous documentation in Epic EHR systems while adhering to HIPAA compliance
- Develop patient education materials for chronic disease management and preventive care
- Participate in continuous quality improvement initiatives for triage protocols
Qualifications
- Active Texas RN license with minimum 3 years emergency department experience
- BLS/Certification; ACLS/PALS preferred
- Expertise in telephone triage using Schmitt-Thompson protocols
- Advanced proficiency in Epic EHR and telehealth platforms
- Exceptional critical thinking and de-escalation skills
- Bachelor of Science in Nursing (BSN) required, MSN preferred
