As a Triage Nurse, you will manage high volumes of incoming calls, assess symptoms with precision, and facilitate seamless transitions between home care and the emergency department. Your expertise will be the backbone of our clinical decision-making process.
Responsibilities
- Perform comprehensive telephone triage assessments for patients presenting with acute symptoms.
- Apply clinical decision-making protocols to prioritize patient care and determine appropriate levels of service.
- Coordinate patient admissions to the Emergency Department and facilitate discharge planning.
- Collaborate with physicians, nurses, and social workers to ensure continuity of care.
- Document all patient interactions accurately and timely in the Electronic Health Record (EHR).
- Provide patient education and support to alleviate anxiety during urgent situations.
- Monitor trends in call volume and work with management to optimize staffing.
Qualifications
- Current and active Registered Nurse (RN) license in the state of New Mexico.
- Minimum of 2 years of recent experience in a hospital or clinical triage setting.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Strong proficiency with Electronic Health Records (EHR) systems.
- Excellent communication and interpersonal skills, with the ability to de-escalate tense situations.
- Ability to work in a fast-paced, high-volume environment.
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred.
