Responsibilities
- Conduct comprehensive clinical assessments of incoming patient calls to determine acuity levels and urgency.
- Provide expert guidance, education, and triage instructions to patients regarding symptoms and appropriate care pathways.
- Document all patient interactions accurately and promptly in the Electronic Health Record (EHR).
- Collaborate effectively with physicians, advanced practice providers, and care coordinators to facilitate timely referrals.
- Escalate critical emergencies to emergency services or on-call providers with zero delay.
- Monitor patient outcomes and follow up on care plans to ensure continuity of service.
- Maintain up-to-date knowledge of clinical guidelines and protocols to ensure patient safety.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of Illinois.
- Minimum of 3 years of clinical experience in a hospital, urgent care, or call center setting.
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications required.
- Strong verbal and written communication skills with a focus on patient empathy and clarity.
- Proficiency in using Electronic Medical Records (EMR) systems (e.g., Epic, Cerner) is preferred.
- Ability to remain calm and composed under pressure in a high-volume, fast-paced environment.
