Responsibilities
- Conduct thorough initial patient assessments via phone, email, or in-person triage to determine the nature and severity of symptoms.
- Apply evidence-based clinical protocols to prioritize patient care and direct individuals to the appropriate medical services.
- Educate patients on self-care management, medication instructions, and preventative health strategies.
- Document all patient interactions, assessments, and care plans accurately within the Electronic Health Record (EHR).
- Coordinate seamless referrals to physicians, specialists, urgent care centers, or emergency departments as needed.
- Monitor patient follow-up compliance and outcomes to ensure continuity of care.
- Maintain a professional, empathetic, and calming demeanor when interacting with anxious patients and their families.
Qualifications
- Valid and active New York State (NYS) Registered Nurse (RN) license.
- BLS (Basic Life Support) certification required; ACLS certification preferred.
- Minimum of 1-2 years of nursing experience, preferably in a clinical or triage setting.
- Strong verbal and written communication skills with the ability to explain complex medical information clearly.
- Ability to work independently while managing multiple concurrent phone lines and tasks efficiently.
- Proficiency with EMR/EHR systems (e.g., Epic, Cerner, AthenaHealth).
- Demonstrated ability to remain calm under pressure and make critical decisions quickly.
