Responsibilities:
- Conduct thorough telephone and virtual patient assessments to determine the severity of medical conditions.
- Collect and analyze patient history, symptoms, and medication data to formulate appropriate care plans.
- Utilize provided medical equipment to assist in remote monitoring and diagnostics.
- Coordinate patient referrals to appropriate medical providers or urgent care facilities when necessary.
- Document all interactions accurately and timely in the Electronic Health Record (EHR) system.
- Educate patients on self-care strategies and medication management.
- Collaborate with physicians and nursing staff to ensure continuity of care.
Qualifications:
- Active Registered Nurse (RN) license in Nebraska.
- Minimum of 2 years of nursing experience, preferably in triage, urgent care, or telehealth.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Strong communication skills with the ability to de-escalate sensitive situations.
- Proficiency with Electronic Health Records (EHR) systems.
- Ability to work independently while maintaining strict adherence to clinical protocols.
