Responsibilities
- Assess patient symptoms and medical history to determine urgency and appropriate care pathways
- Provide clear, empathetic guidance to patients via phone, video, or in-person triage
- Collaborate with physicians to develop timely intervention plans for acute cases
- Maintain precise electronic health records and documentation protocols
- Coordinate seamless patient transfers between departments and facilities
- Implement evidence-based triage protocols aligned with national standards
- Mentor junior staff on triage best practices and emergency response protocols
Qualifications
- Active California RN license with unrestricted status
- Minimum 3 years acute care or emergency department experience
- BLS and ACLS certifications required; TNCC preferred
- Proven expertise in rapid assessment and critical decision-making
- Exceptional communication skills with diverse patient populations
- Proficiency in Epic EHR systems and telehealth platforms
- Ability to work flexible shifts including nights, weekends, and holidays
- BSN or higher degree preferred
