Responsibilities
- Assess patient symptoms via telehealth and in-person triage to determine urgency and appropriate care pathways
- Collaborate with physicians to develop treatment plans for acute and chronic conditions
- Maintain accurate electronic health records while adhering to HIPAA compliance standards
- Provide patient education on home care management and preventive health measures
- Coordinate care transitions between emergency departments, specialists, and primary care
- Participate in continuous quality improvement initiatives for triage protocols
- Respond to high-acuity situations with calm, decisive clinical judgment
Qualifications
- Active RN license in Massachusetts with minimum 2 years clinical experience
- BLS and ACLS certifications required; TNCC preferred
- Demonstrated expertise in emergency triage protocols and patient assessment
- Exceptional communication skills for diverse patient populations
- Proficiency with EHR systems (Epic experience strongly preferred)
- Ability to work independently and exercise sound clinical judgment
- Flexibility to work evenings, weekends, and holidays as needed
- Bachelor's in Nursing or related field preferred
