Responsibilities
- Conduct thorough patient assessments via phone or secure portal to determine medical priority.
- Execute appropriate clinical interventions and referrals based on established triage protocols.
- Maintain accurate and up-to-date patient records in the EMR system.
- Provide clear, empathetic health education and self-management instructions to patients.
- Collaborate with physicians and advanced practice providers to ensure continuity of care.
Qualifications
- Valid and active RN license in the State of Texas.
- Previous experience in triage, telehealth, or a clinical call center setting is preferred.
- Current BLS and ACLS certifications.
- Proficiency with Electronic Health Records (EHR) and telecommunication tools.
- Strong decision-making skills and the ability to remain calm under pressure.
