Responsibilities
- Conduct Patient Assessments: Evaluate patient symptoms via phone and secure messaging portals to determine appropriate level of care.
- Medical Decision Making: Provide accurate triage advice, treatment plans, and referrals to physicians or specialists.
- Documentation: Accurately and timely document all patient interactions and clinical findings in the Electronic Health Record (EHR).
- Care Coordination: Facilitate referrals to urgent care centers, emergency rooms, or home health services as needed.
- Health Education: Educate patients on disease prevention, medication management, and wellness strategies.
- Quality Assurance: Participate in quality improvement initiatives and adhere to clinical protocols and regulatory standards.
Qualifications
- Licensure: Current and valid Registered Nurse (RN) license in the State of Texas.
- Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted with relevant experience.
- Experience: Minimum of 2 years of nursing experience, with specific experience in triage, telehealth, or urgent care strongly preferred.
- Communication: Exceptional verbal and written communication skills with a focus on patient empathy and clarity.
- Technology: Proficient in using computer systems, EMR/EHR software, and remote communication tools.
- Critical Thinking: Strong clinical judgment and ability to make sound decisions under pressure.
