Responsibilities
- Conduct Triage Assessments: Evaluate patient symptoms via phone or secure messaging to determine urgency and appropriate care pathways.
- Direct Care Navigation: Guide patients to the correct providers, specialists, or emergency services based on clinical judgment.
- Documentation: Accurately record patient interactions, triage findings, and care instructions in the Electronic Health Record (EHR).
- Collaboration: Communicate effectively with physicians, nurse practitioners, and other care team members to ensure continuity of care.
- Patient Education: Educate patients on self-care strategies and preventative health measures.
- Policy Adherence: Ensure all clinical workflows comply with HIPAA regulations and organizational protocols.
Qualifications
- Licensure: Current and unrestricted Registered Nurse (RN) license in the state of Texas.
- Experience: Minimum of 3 years of clinical nursing experience, with a preference for Triage or Telehealth experience.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certification required.
- Communication: Excellent verbal and written communication skills with a focus on clarity and empathy.
- Technology Proficiency: Comfortable using Electronic Health Records (EHR) systems and telehealth platforms.
- Decision Making: Strong critical thinking skills and ability to make quick, safe clinical decisions under pressure.
