Responsibilities
- Conduct comprehensive phone and telehealth triage assessments to evaluate patient symptoms and determine the appropriate level of care.
- Provide accurate medical advice, symptom education, and treatment recommendations based on established clinical protocols.
- Document all patient interactions, assessments, and care plans in Electronic Health Records (EHR) systems with precision and confidentiality.
- Collaborate effectively with physicians and other healthcare providers to coordinate referrals and specialist consultations.
- Identify and escalate critical medical emergencies or urgent cases to the appropriate medical staff immediately.
- Manage incoming patient inquiries via phone, secure messaging, or email, prioritizing patient safety and satisfaction.
- Stay updated on the latest clinical guidelines and triage best practices to ensure evidence-based care delivery.
Qualifications
- Current and valid Registered Nurse (RN) license in the State of California.
- Minimum of 2-3 years of acute care or triage nursing experience preferred.
- Graduate of an accredited Nursing program (BSN preferred).
- Strong critical thinking skills and the ability to make sound clinical judgments under pressure.
- Excellent verbal and written communication skills with a focus on patient empathy.
- Proficiency in using Electronic Health Records (EHR) systems (e.g., Epic, Cerner).
- CPR and BLS certification required.
