Responsibilities
- Conduct comprehensive patient assessments via phone, video, or secure messaging platforms.
- Evaluate symptoms to determine appropriate level of care and urgency.
- Provide evidence-based medical advice, medication guidance, and health education.
- Coordinate referrals to specialists and facilitate patient appointments efficiently.
- Document all patient interactions and clinical findings accurately in the EHR system.
- Collaborate with physicians and care teams to ensure continuity of care.
Qualifications
- Active and unencumbered Registered Nurse (RN) license in the state of California.
- Minimum of 2-3 years of experience in triage, emergency nursing, or urgent care.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Strong proficiency with Electronic Health Records (EHR) and telehealth platforms (e.g., Epic, Cerner).
- Excellent verbal and written communication skills with a focus on patient empathy.
- Ability to work independently and make critical decisions under pressure.
