Responsibilities
- Assess patient symptoms via phone or secure messaging platforms to determine medical acuity and urgency.
- Provide accurate medical advice, triage protocols, and self-care instructions based on clinical guidelines.
- Coordinate referrals to appropriate specialists, urgent care centers, or emergency departments.
- Document all patient interactions and triage decisions in the Electronic Health Record (EHR) system accurately and timely.
- Monitor patient follow-ups to ensure adherence to treatment plans and improve patient outcomes.
- Collaborate with physicians, nurse practitioners, and other care team members to ensure continuity of care.
- Stay updated on current clinical guidelines and triage best practices.
Qualifications
- Current and unencumbered Registered Nurse (RN) license in the State of Texas.
- Minimum of 2 years of clinical nursing experience, with at least 1 year specifically in triage or urgent care.
- Basic Life Support (BLS) certification and Advanced Cardiovascular Life Support (ACLS) preferred.
- Strong critical thinking and decision-making skills under pressure.
- Excellent verbal and written communication skills.
- Proficiency in using EMR/EHR systems (e.g., Epic, Cerner, AthenaHealth).
- Ability to de-escalate tense situations and remain calm in high-stress environments.
