Responsibilities
- Conduct thorough patient assessments via phone or in-person to accurately evaluate symptoms and medical history.
- Determine the appropriate level of care, including hospital admission, discharge, or referral to specialists.
- Provide clear, concise instructions to patients and families regarding treatment plans and self-care.
- Document all patient interactions and triage decisions accurately in the Electronic Medical Record (EMR) system.
- Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure seamless patient transitions.
- Manage high call volumes efficiently while maintaining a calm and empathetic demeanor.
- Stay updated on clinical protocols and emergency procedures to ensure patient safety.
Qualifications
- Current and active Registered Nurse (RN) license in the State of Pennsylvania.
- Certification in BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) is required.
- Minimum of 2 years of nursing experience, preferably in a triage or critical care setting.
- Strong clinical assessment and decision-making skills with the ability to prioritize care based on urgency.
- Excellent verbal and written communication skills to convey complex medical information clearly.
- Proficiency in using EMR systems (e.g., Epic, Cerner) is preferred.
- Ability to work flexible shifts, including evenings, weekends, and holidays as needed.
