As a Triage Nurse, you will be the first point of contact for our patients, ensuring they receive the right level of care at the right time. You will work in a fast-paced setting, utilizing your clinical expertise to assess symptoms, triage incoming calls, and coordinate care pathways.

Responsibilities
- Conduct Thorough Patient Assessments: Utilize critical thinking to evaluate patient symptoms via phone and triage requests to determine acuity and appropriate care settings.
- Prioritize Urgent Needs: Rapidly assess and route life-threatening emergencies while efficiently managing lower-acuity inquiries to minimize wait times.
- Document Care: Accurately record all triage interactions, patient history, and care instructions into the Electronic Health Record (EHR) system.
- Facilitate Care Coordination: Schedule follow-up appointments, referrals to specialists, and diagnostic testing as directed by the clinical provider.
- Collaborate with Clinical Teams: Communicate complex cases effectively with physicians, nurse practitioners, and emergency department staff to ensure continuity of care.
Qualifications
- Licensure: Current, active Registered Nurse (RN) license in the state of Minnesota (MN).
- Experience: Minimum of 2 years of recent clinical nursing experience in a hospital, urgent care, or medical office setting.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) required; CNII or TNCC preferred.
- Communication: Exceptional verbal and written communication skills with the ability to explain complex medical concepts to patients of all backgrounds.
- Technology: Proficient in navigating electronic medical records and computer systems.
