Responsibilities
- Perform comprehensive patient assessments via phone, secure messaging, or in-person triage to determine urgency of care.
- Apply clinical decision-making skills to prioritize patient needs and facilitate appropriate referrals to physicians or specialists.
- Document all patient interactions, assessments, and care plans accurately and promptly within the Electronic Health Record (EHR).
- Provide accurate health education and self-care instructions to patients and families regarding chronic disease management and acute symptoms.
- Collaborate with physicians, nurse practitioners, and case managers to ensure seamless continuity of care.
- Monitor and manage incoming patient inquiries, ensuring no critical alerts are missed or delayed.
Qualifications
- Current and unencumbered Registered Nurse (RN) license to practice in Indiana (IN).
- Certification in Medical-Surgical Nursing (CMSN) or Critical Care (CRRN) preferred.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications required.
- Minimum of 2-3 years of progressive nursing experience, with specific experience in triage or telephone nursing.
- Strong computer proficiency with EMR systems (Epic, Cerner, AthenaHealth).
- Excellent verbal and written communication skills, with the ability to de-escalate anxious patients effectively.
