Responsibilities
- Conduct Comprehensive Patient Assessments: Evaluate symptoms via phone or secure messaging to determine the appropriate level of care and urgency.
- Prioritize Care Delivery: Differentiate between urgent and non-urgent cases, ensuring critical patients receive immediate attention.
- Accurate Documentation: Record all triage assessments, patient interactions, and care plans accurately within the EMR system.
- Facilitate Referrals: Coordinate seamless transitions for patients requiring specialist care or urgent care facility visits.
- Patient Education: Provide clear instructions regarding medication, home care, and follow-up appointments.
- Crisis Management: Respond to life-threatening emergencies with standard life-saving protocols.
- Quality Assurance: Monitor call quality and adherence to clinical guidelines to ensure optimal patient satisfaction.
Qualifications
- Valid RN License: Must hold a current, unencumbered Registered Nurse license in the state of California.
- Certifications: Current BLS and ACLS certifications are mandatory.
- Experience: Minimum of 2-3 years of nursing experience, preferably in a triage, telehealth, or urgent care setting.
- Communication Skills: Exceptional verbal and written communication skills with a focus on clarity, empathy, and active listening.
- Technical Proficiency: Proficiency in using Electronic Health Records (EHR) systems.
- Critical Thinking: Ability to make quick, accurate clinical decisions under pressure.
- Availability: Flexibility to work rotating shifts, including evenings, weekends, and holidays.
