Responsibilities
- Conduct Patient Assessments: Evaluate patient symptoms and medical history via phone or secure messaging to determine appropriate levels of care.
- Level of Care Determination: Make clinical decisions to route patients to the ER, Urgent Care, or Primary Care based on severity.
- Patient Education: Provide clear instructions on home remedies, medication management, and when to seek further medical attention.
- Documentation: Accurately record all patient interactions, triage decisions, and referrals into our EMR system.
- Referral Coordination: Facilitate seamless communication between patients and their primary care physicians or specialists.
Qualifications
- Education: Graduate of an accredited Nursing program (ASN, BSN, or Diploma) with a current, active RN license in the state of Arizona.
- Experience: No prior triage experience required. Must be a new graduate or LPN with strong clinical knowledge.
- Skills: Exceptional verbal and written communication skills; ability to de-escalate tense situations calmly.
- Technical: Basic computer literacy and familiarity with electronic health records (EMR) is a plus.
- Availability: Must be available to work flexible shifts, including evenings, weekends, and holidays.
