Job Summary:
The Triage Nurse is responsible for evaluating patient symptoms via phone or virtual channels, providing medical advice, and directing patients to the appropriate level of care. This role requires a high level of clinical judgment and excellent communication skills.
Responsibilities
- Perform comprehensive telephone and virtual triage assessments for patients presenting with a wide range of medical conditions.
- Apply clinical decision-making skills to prioritize patient care and determine the appropriate level of service (ER, Urgent Care, or Physician referral).
- Document patient interactions accurately and in a timely manner within the electronic health record (EHR).
- Provide clear patient education, discharge instructions, and self-care guidance to ensure continuity of care.
- Collaborate effectively with physicians, advanced practice providers, and care coordinators to manage patient load and outcomes.
- Manage high-volume call volumes during peak hours while maintaining a calm and professional demeanor.
- Identify at-risk patients and facilitate timely interventions to prevent adverse health events.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the state of Virginia.
- Minimum of 2-3 years of experience in a clinical setting, preferably in Triage, Urgent Care, or Emergency Medicine.
- Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) is required.
- Strong verbal and written communication skills with the ability to de-escalate tense situations and educate patients effectively.
- Proficiency in using Electronic Health Records (EHR) systems and computer applications.
- Ability to work independently and make critical decisions under pressure.
- Strong organizational skills and attention to detail.
