Responsibilities
- Provide direct patient care including bathing, dressing, and feeding assistance.
- Monitor vital signs and report any changes in patient condition to the RN or LPN.
- Assist with mobility, transfers, and ambulation using proper techniques.
- Document patient care activities, intake, and output accurately in medical records.
- Administer medications and treatments as directed by licensed nursing staff.
- Maintain a clean, safe, and orderly environment for patients and staff.
- Communicate effectively with patients, families, and the interdisciplinary care team.
Qualifications
- Certified Nursing Assistant (CNA) license valid in the state of Illinois required.
- CPR/BLS (Basic Life Support) certification current and in good standing.
- Previous experience in a hospital or long-term care facility preferred.
- Strong compassion, patience, and excellent communication skills.
- Ability to stand for extended periods and lift up to 50 lbs.
- High school diploma or GED required.
