Responsibilities
- Provide direct patient care and assistance with Activities of Daily Living (ADLs) such as bathing, dressing, and grooming.
- Monitor and record vital signs, including blood pressure, temperature, and pulse, reporting any abnormalities to the RN.
- Assist patients with mobility, transfers, and ambulation using proper body mechanics and transfer devices.
- Prepare and serve meals according to dietary restrictions and care plans.
- Maintain accurate and timely documentation in patient electronic health records (EHR).
- Communicate effectively with patients, families, and the interdisciplinary healthcare team.
- Ensure a clean, safe, and infection-free environment for all patients and staff.
Qualifications
- Current and active Texas State CNA License required.
- High School Diploma or GED equivalent.
- CPR and Basic Life Support (BLS) certification (American Heart Association preferred).
- Previous experience in a long-term care, hospital, or home health setting preferred.
- Strong interpersonal and communication skills.
- Ability to stand for long periods and lift up to 50 lbs.
- Valid ID and proof of eligibility to work in the United States.
