Responsibilities
- Provide comprehensive telehealth nursing assessments and patient consultations
- Coordinate care plans with multidisciplinary teams for chronic disease management
- Monitor patient progress through digital health platforms and electronic records
- Deliver patient education on treatment plans and medication management
- Collaborate with case managers to address social determinants of health
- Participate in quality improvement initiatives and clinical protocol development
- Maintain strict HIPAA compliance and data security protocols
Qualifications
- Active RN license in good standing (any US state)
- Minimum 2 years of clinical nursing experience
- BLS certification required (ACLS preferred)
