Responsibilities
- Conduct virtual patient assessments and develop individualized care plans
- Provide telehealth consultations for chronic disease management
- Coordinate with multidisciplinary care teams for complex cases
- Document patient encounters in EMR systems with 100% accuracy
- Monitor and respond to patient inquiries via secure messaging platforms
- Participate in quality improvement initiatives and peer reviews
- Maintain HIPAA compliance in all digital communications
Qualifications
- Active RN license in good standing (US state required)
- Minimum 2 years clinical nursing experience
