Job Summary:
As a Triage Nurse, you will utilize your clinical expertise to assess patient needs via phone and in-person interactions, determine appropriate care pathways, and facilitate timely medical referrals.
Responsibilities
- Conduct comprehensive telephone and in-person assessments of patient symptoms to determine the appropriate level of care.
- Evaluate patient acuity and determine the need for immediate emergency services versus routine follow-up.
- Coordinate and facilitate timely referrals to physicians, specialists, and ancillary services.
- Maintain accurate, up-to-date electronic health records (EHR) documenting all triage interactions and care plans.
- Collaborate effectively with physicians, nurse practitioners, and care coordinators to ensure seamless patient transitions.
- Adhere to all organizational protocols, HIPAA regulations, and nursing standards of practice.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of Arizona.
- BSN preferred; Associate degree in Nursing (ADN) with significant experience will be considered.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Minimum of 2-3 years of clinical experience in emergency nursing, urgent care, or telehealth triage.
- Strong critical thinking and decision-making skills under pressure.
- Excellent verbal and written communication skills for patient education.
