Responsibilities
- Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and toileting assistance.
- Monitor and record vital signs, mood changes, and overall health status to update the care plan.
- Prepare nutritious meals and assist with light housekeeping and laundry as needed.
- Provide companionship and engage clients in stimulating conversations and recreational activities.
- Transport clients to medical appointments, social events, and shopping trips using personal vehicle.
- Administer medication reminders and ensure proper documentation of all care provided.
- Respond promptly to client needs and communicate any concerns to the supervising nurse or case manager.
Qualifications
- High School Diploma or GED required.
- CNA or HHA certification is preferred but not required for qualified applicants.
- Valid Arizona Driver's License and reliable transportation with a clean driving record.
- Strong interpersonal and communication skills with a focus on empathy and patience.
- Ability to lift, transfer, and assist patients up to 50 lbs safely.
- Must pass a background check, drug screening, and health assessment.
- Flexibility to work varying shifts, including nights, weekends, and holidays.
