We are looking for a detail-oriented and patient-focused individual to provide high-quality care in a home health setting.
Responsibilities
- Provide direct patient care, assisting with activities of daily living (ADLs) such as bathing, dressing, and grooming.
- Monitor and record vital signs, including blood pressure, pulse, and temperature, reporting any abnormalities to the Registered Nurse.
- Assist with mobility, transfers, and range-of-motion exercises to ensure patient safety and comfort.
- Prepare and serve meals according to dietary restrictions and nutritional needs.
- Maintain a clean, safe, and organized living environment for patients.
- Communicate effectively with patients, families, and healthcare team members regarding patient status and care plans.
Qualifications
- Current, unencumbered CNA license in the state of Oregon.
- Previous experience in home health, long-term care, or hospital settings preferred.
- CPR/BLS certification (American Heart Association or Red Cross) required.
- Strong interpersonal and communication skills.
- Compassionate demeanor with a genuine desire to help others.
- Reliable transportation and ability to pass a background check and drug screen.
