Responsibilities
- Provide direct patient care by assisting with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
- Monitor and record vital signs, including blood pressure, pulse, and temperature, reporting any abnormalities to the RN or LPN immediately.
- Assist patients with mobility and transfers using proper body mechanics and mobility equipment (hoyer lifts, walkers, etc.).
- Provide emotional support and companionship to patients and their families, ensuring a comfortable and safe environment.
- Prepare and serve meals according to dietary restrictions and patient needs.
- Document patient care activities, intake, and output accurately in electronic health records (EHR).
- Communicate effectively with the interdisciplinary healthcare team to ensure continuity of care.
Qualifications
- Current and valid State Certified Nursing Assistant (CNA) license required.
- CPR/BLS certification (American Heart Association) must be maintained and up to date.
- Minimum of 1 year of experience in a hospital, nursing home, or home health setting preferred.
- Strong interpersonal and communication skills with the ability to interact effectively with patients, families, and medical staff.
- Physical stamina to perform demanding tasks, including lifting and moving patients.
- Must pass a background check and drug screening.
- Candidate must be detail-oriented and possess a compassionate demeanor.
