Responsibilities
- Provide direct patient care, including bathing, dressing, and grooming assistance.
- Monitor and record vital signs, such as blood pressure, pulse, and temperature.
- Assist patients with mobility, including walking, transferring, and using walkers or wheelchairs.
- Prepare patients for meals and assist with feeding as needed.
- Report any changes in patient condition or behavior to the RN or Case Manager immediately.
- Maintain a clean, safe, and sanitary environment for patients.
- Document patient care activities accurately in electronic health records.
Qualifications
- Current and active Certified Nursing Assistant (CNA) license in the State of Texas.
- Basic Life Support (BLS) certification is required.
- Previous experience in home health, hospice, or long-term care is preferred.
- Excellent physical stamina and ability to lift and move patients (up to 50 lbs).
- Strong communication skills and a compassionate demeanor.
- Ability to work independently and as part of a healthcare team.
