Responsibilities
- Provide compassionate in-person care to elderly and disabled clients in San Francisco homes
- Monitor vital signs and document health status via our mobile EHR system
- Assist with ADLs (bathing, dressing, mobility) using telehealth guidance
- Coordinate care plans remotely with RNs and family members
- Transport clients to medical appointments (mileage reimbursed)
- Complete digital shift reports and care documentation
- Participate in virtual team huddles and training sessions
Qualifications
- Active California CNA license in good standing
- 1+ years of home health or hospice experience
- Proficiency with smartphones/tablets for documentation apps
- Current CPR/BLS certification
- Reliable transportation with valid driver's license
- Ability to pass background check and TB screening
- Spanish bilingual skills preferred
- Flexibility for evening/weekend shifts
