Responsibilities
- Provide compassionate personal care assistance, including bathing, dressing, grooming, and toileting support.
- Assist clients with daily living activities such as meal preparation, light housekeeping, and laundry.
- Monitor vital signs, medication schedules, and changes in the client's physical or mental condition.
- Accompany clients to medical appointments, social events, and community outings.
- Offer emotional support and companionship to reduce isolation and improve mental well-being.
- Document care activities and report any concerns to the Case Manager or Nursing Supervisor promptly.
Qualifications
- High school diploma or GED equivalent.
- Valid CPR and First Aid certification (or willingness to obtain within 30 days).
- Previous experience as a Home Health Aide, CNA, or Personal Care Assistant is preferred but not always required.
- Strong interpersonal and communication skills.
- Reliable transportation and a valid driver's license.
- Ability to pass a comprehensive background check and drug screening.
