Responsibilities
- Provide personalized assistance with activities of daily living (ADLs) including bathing, dressing, and mobility support
- Monitor vital signs and document patient observations with meticulous attention to detail
- Prepare nutritious meals and manage specialized dietary requirements
- Administer prescribed medications under professional supervision
- Transport clients to medical appointments and social activities
- Maintain clean, safe home environments following infection control protocols
- Collaborate with RNs and therapists to implement individualized care plans
Qualifications
- New York State HHA certification with current license
- Minimum 2 years professional home care experience
- Valid driver's license with clean driving record
- CPR/First Aid certification (or willingness to obtain)
- Exceptional communication skills and empathy
- Ability to lift 50+ pounds and stand for extended periods
- Proficient in electronic health record documentation
- Reliable transportation and punctuality
