Role Overview:
We are looking for a detail-oriented Triage Nurse to serve as the first point of contact for our patients, ensuring they receive the right level of care at the right time. You will utilize your clinical expertise to assess symptoms via phone and video, coordinate care plans, and drive patient satisfaction.
Responsibilities
- Patient Assessment: Conduct comprehensive phone and video triage assessments to evaluate patient symptoms and determine appropriate levels of care.
- Clinical Decision Making: Apply clinical judgment to prioritize patient needs and make appropriate referrals to physicians, specialists, or urgent care facilities.
- Documentation: Document all triage encounters accurately and in real-time within the EMR system to ensure continuity of care and regulatory compliance.
- Collaboration: Collaborate with interdisciplinary healthcare teams to develop optimal treatment plans for referred patients.
- Patient Education: Provide health education and resources to patients and caregivers to support self-management and recovery.
- Operational Support: Manage high call volumes efficiently while maintaining a calm, professional demeanor and ensuring high patient satisfaction scores.
Qualifications
- Licensure: Active Registered Nurse (RN) license in the State of Texas required.
- Experience: Minimum of 2-3 years of clinical nursing experience, with specific preference for triage, urgent care, or telehealth experience.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) required; TNCC preferred.
- Tech Skills: Strong computer proficiency with experience using EMR/EHR systems (Epic, Cerner, AthenaHealth).
- Communication: Excellent verbal and written communication skills with a focus on active listening and empathy.
- Stamina: Ability to remain calm under pressure and make critical decisions in fast-paced, high-volume environments.
